308. ADVANCE APPLICATION FOR LEAVE

308.1. Advance Permission Required

It is the responsibility of the employee to get approval for absence from duty in advance, except in case of emergencies making advance approval impossible.

308.2. Written Request

A written request for leave must be made if it is known in advance that leave is to be taken. The request shall indicate the type of leave to be taken, the number of days and total leave hours to be taken and the date the employee will return to duty. In the case of emergencies, where the request cannot be completed prior to the leave, it shall be requested the work day after the calendar day on which the employee learns of his/her need for leave. The request must bear the approval of the Department Director or supervisor and shall then be submitted to Human Resources for processing to ensure proper leave records are maintained for each employee.
